The Medical Companion Program Coordinator is responsible for developing policies, procedures, reports on data, and tracks trends for the Medical Companion Program. The incumbent will also provide direct patient care as needed which includes Comprehensive Assessment, Completion of the Client Assessment of Need Score (CANS), Case Management Services, Crisis Intervention and provide coverage and support to other PACT SW staff as needed.
