Incumbent serves as a professional leader on a multidisciplinary case management team at the El Paso VA Health Care System. Serves in the community Veterans with severe/persistent mental illness who have frequent or long-term hospitalizations. Provides individually tailored services to meet the needs of the veterans on a one-on-one basis or in a group setting. This vacancy will remain open until filled. The first cut-off date is 02/12/2021. Additional applications will be referred as needed. Duties include, but are not limited to: Participating in the admission/screening process by interviewing clients and/or other stakeholders Completing intake data collection with purpose of establishing treatment for Severely Mentally Ill (SMI) psychiatric disorders in a case management setting Participating in or facilitating selected supportive groups related to the treatment of specific SMI psychiatric disorders, conducting individual counseling and other social work privileged functions independently or under supervision as professional license dictates Conducting psycho educational activities following professionally prepared resource guides Functioning as a primary case manager for a number of Veterans on a prescribed caseload of SMI Veterans Preparing and implementing treatment/recovery plans developed in concert with the incumbent's multidisciplinary team Acting as a liaison between staff, Veteran and stakeholders in the development and review of treatments/recovery plans Participating in periodic review of ongoing treatment of clients under the care of the MHICM team Setting treatment goals and providing measurement-based care such that progress toward treatment goals is monitored regularly utilizing questionnaires or other screening tools, and treatment goals and discharge planning incorporate results of these questionnaires or screenings Providing ongoing documentation of client's progress using a behavioral framework such as but not limited to: using SOAP format where S-subjective input, 0-behavioral observation, A- clinical assessment, P-collaborative plan Other duties as assigned. Work Schedule: Monday thru Friday 8:00 a.m. to 4:30 p.m. Financial Disclosure Report: Not required This is a Bargaining Unit position
