The RN Mental Health Case Manager demonstrates clinical expertise, leadership, and creative approaches to management of mental health nursing care for general outpatient mental health patients with DSM 5 diagnoses within Dorn VA Medical Center and Community Outpatient Clinics (CBOCs). The RN Mental Health Case Manager applies clinical skills of case management which emphasizes a collaborative process that assesses, advocates, plans, implements, coordinates, monitors and evaluates health care options and services so that they meet the needs of the individual patient. The RN Mental Health Case Manager will follow discharged Veterans from inpatient psychiatric unit (VAMC or Community), new patients to the Mental Health outpatient clinic, and those with history medication non-adherence, episodes of crisis including suicidal and /or homicidal ideations/plan and symptoms that have impaired the functioning of the Veteran's activities of daily living. In addition to face to face in person therapeutic/patient educational sessions, will engage in technology of VVC and telemedicine as deem appropriate. Will also be apart of the Long Acting Injection Clinic and share responsibilities with medication administration with two other peer RN Mental Health Case Managers. Work Schedule: Monday - Friday, 8:00 am to 4:30 pm. Subject to change based on the needs of the agency. Financial Disclosure Report: Not required
