This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Geriatrics and Extended Care Service line of the Northern California VA Health Care System is currently recruiting a Social Worker (Program Coordinator) for the Homemaker/Home Health Aide (H/HHA) program at the Fairfield location. Major duties include, but are not limited to: Participate in the development, evaluation, and communication of standards, policies, and procedures, and plans of care for the H/HHA program. Educate medical center colleagues and/or students regarding the H/HHA program and assist with dissemination of information to other VA facilities and community partners. Evaluate H/HHA practices using relevant statutes and regulations and take action to improve identified deficiencies. Track H/HHA program assessments, consults, authorizations, statistics, data management, identify significant findings, problem solve, complete documentation reports pertaining to caseloads, productivity, and trainings. Complete an initial screening of Veterans to determine whether they meet eligibility criteria for inclusion in the H/HHA program. Reassess Veterans on a regular basis to ensure they are receiving the appropriate level of services and remain in need of program support. Serve as a liaison between the contracted H/HHA provider and Veteran and/or family, which includes initiating services, reducing or increasing authorized hours, addressing conflicts between the Veteran and/or family and agency, and/or terminating services. Review agencies which may be used to provide in home services to Veterans. Make rapid assessments and develop crisis management plans to maintain a patient in the home or for admission to acute, short term, and/or long-term placements. Provide education related to VA and community resources, entitlements, Advance Directives/Living Will, Life-Sustaining Treatment decisions, and will refer Veterans and/or families to other GEC programs or appropriate VA resource for identified health education needs. Coordinate referrals to non-institutional services in collaboration with team members. Provide Veterans and their caregiver's ongoing supportive counseling. Preferred experience: Strong administrative coordination skills Excellent knowledge of Microsoft Office software programs (especially excel). Experience with Home Health Agencies Work Schedule: Monday Through Friday 8:00 Am to 4:30 PM Financial Disclosure Report: Not required Telework, if approved, would be on a regularly scheduled basis, but this is not a virtual or remote position.
