The Greater Los Angeles VA Healthcare Network is recruiting for a Senior Social Worker in the Homeless Patient Aligned Care Team (HPACT) clinic in Community Engagement and Reintegration (CERS) for its Sepulveda location. The goal of the Senior Social HPACT clinic is to improve continuity of care and coordination of services for Veterans who are either homeless or at risk for homelessness by working with existing clinical and community care programs. The Senior social worker functions as a member of the Homeless Patient Aligned Care Team. As a member of the team, the social worker will focus on housing issues, securing housing, using available homeless resources, prevention of homelessness, health promotion, connecting and coordination of multiple services related to care needs, identification of at risk for homeless Veterans. The major duties and responsibilities include but are not limited: Assesses and treats the complicated psychosocial problems of Veterans, families, and significant other. Provides assessment of homeless and other psychosocial problems that cause distress, can include adjustment to the current medical condition and/or functional status, untreated or under- treated mental health or substance abuse condition, economic instability, legal problems, and inadequate social supports, housing and transportation. Engages in case management services with members of the outpatient interdisciplinary team to meet the agreed upon treatment goals. Participates in improving chronic disease outcomes by providing primary and secondary preventive care services and self-management tailored to homeless Veteran needs. Assists in increasing housing placement, retention, and maintain supportive relationships with homeless Veterans during critical care. Conducts comprehensive assessments of Veterans and develops treatment plans that are sensitive to Veterans' diverse needs and abilities. Provides individual, group, and family psychotherapy and advanced level case management interventions, including but not limited to: participating in treatment groups that support health promotion such as smoking cessation, MOVE and shared medical appointments, and using motivational interviewing skills to encourage Veterans to incorporate healthy behaviors into their lives. Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services.
