| Bay Pines VA Medical Center is a general medical and surgical facility, classified as Level 1A Complexity. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology, as well as education and limited research. Providing comprehensive healthcare through primary care, mental health and long-term care. In addition to the main facility in St Petersburg, there are services offered in seven community-based outpatient clinics and one outpatient clinic. This position is located in Social Work Service of the Bay Pines VA Healthcare System. Incumbent will provide services at the Lee County Healthcare Center, Cape Coral, FL. The duties and responsibilities are carried throughout the HealthCare Center including all clinical and other patient care areas involved with the services. Clinical functions include: Provide case-appropriate professional social work services for Veterans and families/significant others in compliance with VA, VISN, station, and Social Work policies and memorandums. Incumbent provides highly developed professional social work services. Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Must utilize knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups in the outpatient mental health clinic. Social Workers must practice within the bounds of their license or certification. Must have knowledge of human development and behavior (physical and psychological), and the differential influences of the environment, society, and culture. Independently assesses the psychosocial functioning and needs of individuals and families from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds and is able to formulate and implement a treatment plan, identifying the patient's problems, strengths, needs, abilities, preferences, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team. Works with patients and families who are experiencing a basic emotional, medical, and social problems, utilizing individual, group, and family counseling skills. Uses basic knowledge of psychosocial treatment modalities and ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. Uses judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Establishes and maintains effective working relationships with clients, staff, and representatives of community agencies. Communicates effectively, both orally and in writing, with people from varied backgrounds. Serves as liaison to community agencies and networks with agency representatives. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Establishes and maintains effective therapeutic relationships with Veterans and their families. Independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. Independently identifies high-risk patients and provides case management services. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Utilizes working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. May serve on committees, councils, workgroups, and/or task forces. He/She obtains and/or participates in continuing education. Independently provides consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Administrative functions include: Organizing work, setting priorities, meeting multiple deadlines and evaluating assigned program area(s). Carrying full responsibility for cases, presenting a wide range of social, emotional and environmental problems and will provide services according to the assessment of the Veteran's needs, encompassing even the most difficult cases. Providing training, orientation, consultation and guidance within clinical specialization of practice. Assume personal responsibility for continuing professional education (per NASW Standards for Continuing Professional Education. Comply with the fiscal year (mandatory training requirements of the Health Care System. May serve on committees, councils, workgroups, and/or task forces. He/She obtains and/or participates in continuing education. Professionally responsible to learn, practice and support all existing safety practices relevant to their position. The incumbent will further contribute to and keep abreast of new safety practices as they are developed. Provides in-service training to Social Work Service staff, to staff from other disciplines, and to students and volunteers as assigned. The incumbent supervises and/or trains MSW graduate school interns, summer aides, and volunteers as assigned. Work Schedule: Monday - Friday, 8:00am - 4:30pm Financial Disclosure Report: Not required
