This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The New Mexico Veterans Affairs Health Care Systems (NMVAHCS) Behavioral Health Care Line (BHCL) is looking for (2) Senior Social Workers who will be assigned to the Psychiatry Primary Care Clinic (PPC). They will be apart of an interdisciplinary team providing psychiatric, primary care and case management to veterans with chronic mental illness.The position involves clinical and administrative duties in PPC. Veterans enrolled to PPC are a special population with highly complex health and mental health problems requiring expert level psychosocial interventions Major duties include, but are not limited to: Identifies high-risk patients and provides case management services. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Conducts psychosocial assessments and develops treatment/recovery plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Provides consultation services, as subject matter expert, to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan. Coordinates the patient flow and access within the clinic including functioning as the local area PCMM Coordinator with responsibility for assignment of PCP and Treatment Coordinators within the clinic, coordinating the Ward 7 follow up appointments, monitoring SAIL measures including continuity of care, experience of care and Veterans loss to follow up. Develops innovations in practice interventions, and provide clinical supervision for social work licensure or certification.
