The REACH VET Coordinator (RVC) works for the Mental Health Service at the Hershel "Woody" Williams VA Medical Center. The Reach Vet Coordinator will serve as part of the Suicide Prevention team to implement a mandated program using predictive modeling to identify Veterans at high statistical risk for suicide and other adverse outcomes. Duties may include but are not limited to the following: Demonstrate leadership by carrying out the decisions of the REACH VET program, adjusting to change or work pressure in a pleasant manner, and engages in problem solving to resolve conflicts and differences of opinion in a productive, professional manner; Provide consultation to mental and medical health staff on the rationale, utility, and steps associated with REACH VET including clinical direction with collaboration with provider(s), service line supervisors, and mental health leadership; Maintain access to the REACH VET dashboard to receive names of newly identified Veterans; Track and collect data to then present to Hospital leadership to show Program progress; Documenting, when appropriate, the nature of follow-up plans for continuing treatment in the electronic medical record; Provide case management services to include internal and external referrals, resources, education, advocacy, and supportive services; Other duties as assigned.
