The Iowa City VA Health Care System (ICVAHCS) is currently recruiting for one (1) Full-Time Registered Nurse - Care Coordination Case Manager. ICVAHCS is in Veterans Integrated Service Network (VISN) 23. The Care Coordination Case Manager Registered Nurse falls under the clinical pathway, as a direct care nurse and reports to the Clinical Manager, Community Care Service Line. The Registered Nurse - Care Coordination Case Manager is responsible for coordinating episodic care of Veterans, who are seeking treatment using a community provider or need urgent referral to community services. Duties include but are not limited to: Managing responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. responsible for supporting the Veterans Access to Care Act and the VA MISSION ACT OF 2018. Reviews notifications/community care consults and patient records to determine compliance with established VHA criteria for clinical authorizations. Determines the most appropriate avenue for care to be administered, within the VA Iowa City Health Care System or the private sector. Coordinates Community Care with the VA Iowa City Health Care System, primary care teams and/or specialty clinics. Accountable for assessing, planning, and implementing patient care at a patient level. Works within the framework of the philosophy of the VA Iowa City Health Care System and collaborates with clinical and ancillary staff, to participate in programs to enhance the delivery of patient care. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Work Schedule: Monday - Friday, 8:00AM - 4:30PM.
