Licensed Practical Nurse (LPN) will be providing care to veterans located in the Homeless Pact team and various outpatient mental health clinics and/or community settings Domiciliary, Transition, IOP, Medication Clinic, MHICM, SUD, Mental Health Clinics. As an LPN he or she is self directed and requires minimal supervision. The incumbent has demonstrated skill sufficient to independently resolving a range of nursing problems. Also, he or she provides care in compliance with program and regulatory guidelines (JCAHO, CARF, etc.) and VHA/Nursing Strategic Plan Provides instructions on physical limitations or enhancements of physical activity to promote better outcomes for the patient. Performs assigned technical procedures after training; e.g., weights, blood pressures, pulse, temperatures, accu-checks. Prepares and administers medications, as outlined by Nursing Service medication administration and program policy and procedures. Assists with diagnostic or therapeutic procedures. Participates in the admission, transfer and discharge of the patient. Provides for the safety and comfort of the patient. Practices universal precautions. Recognizes emergency situations, summons appropriate help and responds appropriately. Provides for continuity of care in instructing patients about equipment, laboratory needs, and procedures to be performed prior to next visit, and explains to patient and their family's medication instructions and how medicine can affect disease processes. Checks the crash cart, AED and other assigned equipment Monitors medication, specimen and nourishment refrigerator temperature. Contributes to interdisciplinary care planning conferences. Contributes subjective and objective data to the nursing care regimen. Notifies supervisory staff nurse and/or supervisor of any problems or changes in patient's condition. Notifies charge nurse/nursing supervisor when there are large spills, linens needs, maintenance & supply issues etc. Completes appropriate documentation and forms. Participates in unit based quality improvement activities to positively impact care Instructs patient on procedures to be performed; e.g., CT scans, Doppler study, ultrasound, etc. PACT related duties, venipuncture, bladder scans and other procedures in primary care mental health settings after training and competency verification Work Schedule: 7:30 am - 4:00 pm, Monday - Friday, rotating weekends, holidays, and off tours (This position is offsite) Financial Disclosure Report: Not required
