The incumbent is located at one of the sites of care for the VA Central California Health Care System, including the main Fresno campus, the Community-Based Outpatient Clinics (CBOCs) in Oakhurst, Merced, and Tulare. Future locations may be added or deleted based on organizational need. The work involves the independent performance of the full range of functions using a variety of conventional techniques and procedures. The work involves a variety of patients of diverse cultural, spiritual, and education backgrounds. The work directly affects the overall well-being of patients served. The work involves the independent involvement of scheduled appointments following accepted facility practices and national scheduling directives. The employee must be able to identify materials and techniques used with each patient. Guidelines include operating policies and practices of the facility including those involving telehealth; accepted materials, procedures and techniques associated with each piece of equipment for telehealth; and other facility publications including memoranda of understanding, service agreements, and telehealth plans. Equipment for telehealth includes items such as clinical video teleconferencing (CVT) carts. The employee uses judgment in selecting and adapting techniques, procedures, and materials most appropriate for the specific condition of each patient in an independent manner. Materials may include components of the telehealth equipment or educational materials for the providers, clinic staff, or patients. Educational material addresses the process of telehealth, required follow up, and roles and responsibilities of the remote providers and the local provider and patient. Situations requiring deviations from the guidelines are referred to the supervisor. Duties include: Incorporate patient care data by actively contributing to the development of the patient's overall health care plan Carry out assigned nursing interventions Develop and modify treatment plans Assist RN and/or MD/DO to prioritize overall patient care needs Adjust plans for care delivery Make astute observations of subtle, less obvious changes, both physical and emotional, in patient's condition, and accurately and thoroughly document care delivered and patient's response to care that may require immediate modification of the patient's care plan Seek assistance and guidance by informing RN or MD/DO of changes in patient's condition requiring higher-level intervention. Participate to a greater degree in prioritizing, and, as appropriate, modifying patient care treatment Possess the ability to observe more subtle physical and emotional changes in patients and demonstrate taking appropriate action based on those observations Possess knowledge and skill with respect to these responsibilities than employees at lower grade levels Promptly recognize potential urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO for appropriate team intervention and effectively intervene, as directed, to assure the continuation of optimal, safe, and therapeutic patient care delivery Knowledge of the patient treatment process and exercise judgment that demonstrates the ability to effectively intervene when appropriate Assists individuals or groups of patients and their families to take an active role in promoting healthy lifestyles for more positive patient outcomes Exercise skill in influencing and communicating with unusually difficult to care for or communicate with patients and/or family members who may exhibit problems with lack of self-control, resistant or abusive behaviors, or impediments in their ability to understand or follow instructions Ability to effectively deal with the above types of patients under very general supervision Actively promotes and effectively incorporates established customer service standards in all aspects of their practice Possesses comprehensive practical nursing knowledge and skills in specialized procedures and practices to serve as a resource person for other staff in organizing and implementing more complex/specialized patient care Promptly reports any deviations from normal practice patterns to the RN or MD/DO. The staff mix is such that employees at this grade level are expected to perform practical nursing duties of considerable difficulty and responsibility under very general supervision Actively seek out educational opportunities to enhance nursing knowledge and skills, and providing leadership Work Schedule: Monday- Friday 8:00 a.m.-4:30 p.m. Financial Disclosure Report: Not required
