The Greater Los Angles Healthcare Network is recruiting for two (2) Senior Social Workers, for the Community Engagement & Reintegration Service (CERS), within Housing & Urban Development Veterans Affairs Supportive Housing (HUD-VASH). One (1) position is located at the San Luis Obispo Veteran Affairs Clinic, and one (1) position is located at the Bakersfield Veteran Affairs Clinic. The incumbent executes case management responsibilities for the HUD-VASH program. The Senior Social Worker (HUD-VASH) is responsible for providing the appropriate treatment and services to all homeless Veterans in the HUD-VASH program. The major duties and responsibilities include but are not limited to: Addresses psychological, emotional, and social needs of homeless Veterans. Provides clinical and administrative services for the HUD-VASH program. Identifies the impact of psychosocial problems on health care, and the adherence to treatment plans. Provides medical and mental health diagnoses, disabilities, and treatment procedures, for Veterans in the HUD-VASH program. Coordinates and provides referrals to programs, and services for Veterans. Provides acute, chronic, and traumatic illness treatment to Veterans. Works independently with patients and families to treat a variety of psychiatric, medical, and social problems. Provides information pertaining to Veterans' benefits, special programs, service-connected compensation, and non-service connected pension to Veterans in the HUD-VASH program. Travels into the community to conduct interviews with the veterans, their families, representatives of community health and welfare agencies and law enforcement agencies.
