The Community Care Nurse Case Manager works collaboratively with all clinical divisions across the VA Portland Health Care System (VAPORHCS); in addition, he/she works collaboratively with the administrative staff within the Community Care Program to ensure coordinated, timely, cost-efficient care to the Veteran. The Community Care Nurse Case Manager is responsible for the clinical case management activities necessary to purchase patient care in the community in a manner that successfully integrates the community care with past and future VA care. This includes, but is not limited to: monitoring of patients hospitalized in non-VA facilities identification of patients appropriate for community care services triage of community care consults coordination of care for the patient with the VA referring provider and the community provider collaboration with the authorization staff to maintain fiscal responsibility and efficiency integrating the patient back into the VA system when community care episode is completed The Nurse Case Manager frequently acts on behalf of VAPORHCS with other VISN20 sites to determine availability of clinical services, ensure proper transfer of patient care, participate in collaborative case management planning, utilization review and other related case management activities. Based on education and experience, the Nurse Case Manager may be assigned responsibility as preceptor and/or charge nurse.
