The Community Care Social Worker provides case management for Veterans receiving Care in the Community in a hospital, long term care, or residential setting. The Social Worker is organizationally located in the community care department at the VA Finger Lakes Healthcare System. Incumbent is a professional Social Worker whose duties and responsibilities are assigned to the care management of Veterans in the Community Care Program. Provides Community Care program support for the Finger Lakes staff in their comprehensive psychosocial and case management services to Veterans placed in Community inpatient settings. Has a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures. Must have a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans in the contracted Community Care Programs. Must independently complete thorough annual psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families who have Community care services. Must actively participate as a member of the interdisciplinary treatment team by collaborating with treatment team members in the inpatient units and Community Living Centers (CLC). Must actively participate through collaboration with Veterans and family as well as Community Care Program and interdisciplinary treatment team members in the development and implementation of a Veteran-centered treatment plan. Possesses a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, common medications and their effects/side effects, and medical terminology. Facilitate transfer coordination and/or discharge planning through collaboration with Veterans and their families as well as Community Care Program vendors and interdisciplinary treatment team members. Serves as a liaison between Veterans, their families, VA providers, and community resources in order to ensure thorough delivery of services. Possesses knowledge and ability to independently implement treatment modalities, provide counseling and/or support for Veterans and families. Provides consultation and education to Veterans, their families, and contracted vendors regarding community resources, VA benefits and specialty programs, and advance directives Must be available to provide consultation to other treatment team and staff members as well as vendors regarding psychosocial needs of Veterans and/or their families Maintains knowledge of Veteran's benefits and services, community resources, and process for making appropriate Community Care referrals to community and to other governmental programs or agencies. Possesses skills and knowledge to provide case management services to Veterans and their families throughout the continuum of care. Possesses and demonstrate ability to communicate effectively, both orally and in writing, with people from varied backgrounds. Willing to serve on committees, work groups, and task forces at the facility as deemed appropriate by the Community Care supervisor and Social Work Executive. Responsible for furthering professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment and maintenance of state Social Work license. Able to aid with the orientation and learning of less experienced Social Workers to serve as a preceptor for Social Work students. Able to maintain a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Joint Commission standards, The Office of Community Care standards, and facility standards.
