The Iowa City VA Health Care System (ICVAHCS) is currently recruiting for one (1) Full-Time Registered Nurse- Mental Health Outpatient Clinic Case Manager for our Davenport, Iowa Community Based Outpatient Clinic (CBOC). ICVAHCS is in Veterans Integrated Service Network (VISN) 23. The Mental Health Outpatient Clinic Case Manager Registered Nurse falls under the clinical pathway, as a direct care nurse, and reports to the Mental Health Service Line leadership. The Registered Nurse- Mental Health Outpatient Clinic Case Manager (RN MHCM) functions as a case manager, with advanced clinical knowledge and competence to function in a variety of roles in the clinic setting, while providing collaborative care to assigned Veterans or Veterans who present to the clinic, as needed. Duties include but are not limited to: Knowledge of mental illness as it relates to: case management, assessment, current treatments, key symptoms, patient manifestations, and medications prescribed for treatment, patients responses to medications and possible adverse reactions. Obtains health history of referred/enrolled patients including conducting assessments/reassessments of mental illness symptoms, and assisting in the development and implementation of patient's treatment plans in collaboration with their caregivers and primary care mental health providers. Provides patient education in areas of mental illness management and understanding, health promotion, and functional restoration. Provides consultation, guidance, and acts as a liaison to staff, nursing homes, home health agencies and other health care providers regarding patient care needs in the community/home setting and in the interpretation of VA rules and regulations. Accountable for utilizing critical thinking skills and sound clinical judgement to ensure positive patient outcomes; for demonstrating ethical practice; and for participating in performance improvement activities to improve the quality of care and ensure performance measures are met and/or exceeded. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self- management after discharge, and supporting factors that impact customer satisfaction. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.
