The Long Beach VA Healthcare Network is recruiting for 3 positions as a member of a multidisciplinary Housing and Urban Development Veterans Affairs Supportive Housing (HUD VASH) team and is responsible for coordinating and providing individual and family counseling and case management services. S/he conducts assessment interviews, develops treatment plans and provides treatment to eligible Veterans and their families. The major duties and responsibilities include but are not limited to: Provide a full range of social work services within commonly accepted standards of social work practice which Includes case work, psychosocial assessments, individual, family and group counseling treatment planning, discharge planning, address Advance Directives, and other services as appropriate. Perform site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes; verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation and is responsible for ensuring that homeless veterans are considered as primary participants for program acceptance. Coordinate care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Participate in program planning and development, advising he supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Coordinate with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program veteran.
