The Senior Social Worker is the Substance Use Disorder (SUD) Specialist for the Gold Team. The SUD Specialist supports the Gold Team by providing engagement and as needed, ensuring the continuity of care between the different subprograms of the Gold Team. The Gold Team provides intensive case management and psychotherapy for Veterans living with substance abuse disorders and frequently additional mental health illnesses with the goal of harm reduction and recovery. The Senior Social Worker is the Substance Use Disorder (SUD) Specialist for the Gold Team. The SUD Specialist supports the Gold Team by providing engagement and as needed, ensuring the continuity of care between the different subprograms of the Gold Team. The Gold Team provides intensive case management and psychotherapy for Veterans as they transition from engagement and stabilization through programs such as Substance Abuse Rehabilitation and Treatment Program (SARRTP), Intensive Outpatient Program (IOP), Outpatient Program (OP), and as appropriate, Opioid Replacement Clinic (ORC). The SUD Specialist provides ongoing early substance use treatment interventions, substance use case management, intensive case management, and substance use psychotherapy to promote harm reduction to sustained abstinence. The Senior Social Worker will function as the Liaison between the various Gold Team programs and other VANTHCS Mental Health Service programs to ensure appropriate coordination of care, and transitions of care for enrolled Veterans pertaining to SUD. The Senior Social Worker provides SUD supportive care, educational and psychotherapeutic groups. Duties include but are not limited to: Perform specialized treatment of complex physical or mental illness as it relates to mental health illness, severe mental health illness and comorbid substance use disorders. Incumbent incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved scope of practice. Incumbent has the ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs of substance use disorders. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program. Develop and implement methods for measuring effectiveness of social work practice and services, utilizing outcome evaluations and use of evidence-based treatment modalities that are effective with Veterans who struggle with substance use disorders along with medically and psychiatrically complex conditions. Incumbent also provides specialized assessments, and treatment planning, as well as SUD consultation, to other team members to provide interventions and strategies for engagement for difficult to engage clients. Independently conduct comprehensive substance use assessments and develops treatment plans in collaboration with Veteran, family, and interdisciplinary treatment team. Monitor e-consults, complete SUD assessments, and make recommendations to the Gold Team in addition to providing direct clinical care to Veterans struggling with SUD concerns. Develop and maintain effective partnerships with other VANTHCS teams and programs and follow leadership guidelines regarding access to VA substance abuse programs. Routinely participate in all Gold Team team meetings related to Veterans enrolled in substance use disorder programs to ensure good transitions of care for population served. Develop and provide ongoing training and in-services to multidisciplinary team members in area of expertise.
