The Social Worker functions as a Discharge Planner for the Substance Abuse and Psychosocial Residential Rehabilitation Treatment Programs (SARRTP/PRRTP) at the VA Nebraska Western Iowa -Omaha VAMC. The Social Worker will function as part of a multidisciplinary team, and will independently assess, evaluate and treat veterans with addictions and mental health disorders. The incumbent is responsible for completing the following duties/responsibilities: Conducts suicide risk assessments and safety plans upon admission to the RRTPs. Identifies aftercare needs that factor into the development of a treatment plan. Actively participates in the treatment planning process with other disciplines and will assist with such. May make recommendations and suggestions for any veteran in RRTP. Obtains and interprets information related to substance use disorders and mental illnesses, as well as biological and sociocultural dimensions influencing these. Provides discharge planning to veterans in the residential programs, with the goal of facilitating their transition to appropriate outpatient resources and services. Works with internal and external stakeholders to facilitate discharge plans. Develops relationships with other departments and agencies in order to facilitate this. Conducts short-term therapeutic counseling as part of discharge planning sessions, with an emphasis on motivational enhancement. Serves as a therapist/counselor for group therapy sessions. Observes behavior and document interactions appropriately. Conducts lectures and makes educational presentations to veterans related to various aspects of substance use and dependence. Makes presentations to community organizations, therapeutic groups, special interest groups and students on substance use and dependence related topics as needed. May provide consultation to physicians and other professionals within the medical center to assess a veterans appropriateness for substance use disorder treatment and what level of treatment is most appropriate.
