The Polytrauma Coordinator (Social Work Case Manager) facilitates and coordinates case management programs that include intake, assessment and evaluation of referrals for Polytrauma rehabilitation services for active duty service member and Veterans who have experienced severe injuries resulting in multiple traumas, including spinal cord injuries; traumatic brain injuries, visual impairment, amputations, combat stress, and post-traumatic stress disorder. The incumbent will collaborate with Chief, Physical Medicine and Rehabilitation/ designee and members of the Polytrauma Support Clinical Team when assigning case management. The incumbent will also provide ongoing comprehensive social work interventions to returning combat service members, Veterans and their families over the course of time VHA health care services are being. provided. The Polytrauma Coordinator (a social worker master's degree practitioner) independently provides, clinical psychosocial and case management services at an advanced practice level to eligible active duty service members, Veterans and their family members to` meet biological; psychological, social, and environmental needs. Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight, coordination and creative problem-solving. Services are provided in healthcare settings, both inpatient and ambulatory, and in the community. The incumbent works closely and collaborates with Chief, Physical Medicine and Rehabilitation (PM&R) Services, Polytrauma Support Clinical Team and the Miltary2VA (M2VA) Program Manager. The incumbent will provide program coordination but not supervision to the case managers assigned to the Polytrauma Program.
