This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more This position is located within the Department of Veterans Affairs (VA), Veterans Health Administration (VHA), VA Mid-Atlantic Health Care Network (VISN 6), Hampton Veterans Affairs Medical Center (HVAMC), Portsmouth Community Based Outpatient Clinic. This position is aligned under the Primary Care Mental Health Integration Program (PCMHI) and Behavioral Health Laboratory (BHL) Hub, which is administratively aligned under the Mental Health and Behavioral Sciences Service line. The incumbent functions as part of multidisciplinary patient aligned care teams (PACTs), and delivers brief, consultation-based services to Veterans within the primary care service context. This individual will function as a full member of the PACT and associated specialty medical team(s) and provide ongoing team consultation services to other PACT and associated medical team members to support their care of PACT patients. Responsibilities include assessing for psychopathology, screening for cognitive deficits and problematic health behaviors, and brief psychotherapy. The individual will also provide interventions to help patients manage behavioral factors associated with behavioral medicine problems and to promote medical compliance. Mental health treatment and prevention-oriented services will emphasize evidence-based and best practice approaches. The incumbent may also be expected to provide support to PACT, Primary Care, and/or Mental Health through committee or workgroup participation; outreach activity support, staff education; and policy and program development support. Services are provided in the form of individual or group face-to-face, telecommunication and Clinical Video Telehealth (CVT) visits. Visits may be provided with PACT team members or the Veteran, or in joint meetings with the Veteran and other PACT providers. In either case, the Veteran's care providers will develop a common conceptualization of the Veteran's health and mental health problems and their interrelationships, which all team members will incorporate into their individual assessment and treatment services.
