The RN functions as a care manager for the HBPC assigned veterans, providing care in veterans' homes and is responsible for assessing and continually reassessing the needs of veterans and caregivers. Delivering nursing care in the home. Delegating and assessing the care by LPNs or vocational nurses, and home health technicians, if applicable Duties for this position are but not limited to the following: Advance clinical knowledge and skills with focus in primary, geriatric and/or home care. Ability to assess patient's learning needs, development, and implementation of nursing care plans to meet their needs, while providing education for patients and families in self-care and management of chronic conditions. Demonstrates leadership in delivering and improving holistic care utilizing collaborative strategies with patients, family members/significant others, and members of the interdisciplinary team Skill in being a part of an interdisciplinary team while acting in the role of case-manager for an individual caseload of patients. Requires initiative, autonomy and ability to collaborate while providing effective planning and execution of patient care to facilitate the patient's transition into HBPC program. The nurse works with many disciplines and different services to coordinate the patient's care. Work Schedule: Monday through Friday, 7:30AM to 4:00PM or Four 10 hour shifts (No Monday or Friday off) Compressed/Flexible: Available Telework: Available Virtual: This is not a virtual position. Paid Time Off: Annual leave at the rate of eight (8) hours per biweekly pay period; Sick leave at the rate of four (4) hours per biweekly pay period; 11 paid Federal holidays per year. Retirement: Traditional federal pension (5 years' vesting) and federal 401K with up to 5% in contributions by VA. Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement). Licensure: 1 full and unrestricted license from any US State or territory.
