The incumbent serves as the Social Work Program Coordinator for the Palliative Care at the Eastern Oklahoma VA Medical Center and works closely with the Palliative and Hospice team to provide care to Veterans and their families based upon identified need. Some of the required duties are, but not limited to: The incumbent is a member of a diverse, inter-professional team who works independently and has extensive experience developing and leading groups, partnerships, or coalitions addressing palliative care. The incumbent is the social work subject matter expert for Palliative/Hospice Care more than 25% of the time. Organizes, sets priorities meets multiple deadlines and evaluates in conjunction with the Palliative and Hospice team. Prepares statistics and reports for presentation to the VISN Geriatric Extended Care (GEC) Liaison, as requested. The Palliative Care Social Work Program Coordinator is responsible for program coordination and administration. Reviews all applicable social work consults and ensures timely responses within prescribed timeframes. The incumbent disseminates relevant research findings through practice, education, and consultation with non-VA community partners as well as other VA health care professionals and administrators. The incumbent comprehends and maintains knowledge of current VA guidelines, policies, Federal regulations, and laws governing benefits for Veterans. Serves the psychosocial needs of patients, their families and caregivers through assessment, intervention and treatment planning as the Palliative Care clinical social worker. The incumbent demonstrates effective public speaking skills and the ability to communicate effectively, both orally and in writing, with a variety of audiences. The incumbent fosters strong collaborative relations with multidisciplinary staff members and members of community partner agencies to develop and implement palliative and hospice educational programs and initiatives. The incumbent demonstrates strong interpersonal and collaborative skills, working with staff and stakeholders at all knowledge and proficiency levels. The incumbent has working knowledge of word processing, spreadsheet, and audiovisual presentations software such as Microsoft Word, Excel, and PowerPoint. The incumbent performs statistical reporting, evaluation, consultation, and collaboration with non-Department of Veterans Affairs (VA) community partners as well as other VA health care professionals and administrators. The incumbent comprehends and maintains knowledge of current VA guidelines, policies, Federal regulations, and laws governing medical benefits for Veterans.
