This position is located in Social Work Service, Boise VA Medical Center, Boise, ID. The social worker is responsible for the management, coordination and provision of social work services to veterans and their families on the Inpatient Psychiatry Unit at the Boise VA Medical Center. The incumbent is responsible for completing psychosocial history and assessments, advance directives, treatment planning, case management, individual, couples and group counseling, and coordination of discharge plans for veterans admitted to the Inpatient Psychiatry Unit. Services are focused upon facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence and functioning possible, gaining the cooperation of relatives in the treatment of the Veteran, and planning jointly with the Veteran and family or collaterals for the fullest use of community resources to effect rehabilitation and satisfactory social adjustment. In all phases of Social Work delivery, the social worker determines and uses appropriate interview, counseling, and treatment techniques. Major duties will include, but are not limited to: Conduct psychosocial assessments to determine needs of veterans and family members. Is an active participant in the treatment planning process with other disciplines. Participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. Provide individual, family and group counseling as appropriate. Provide consultation/education to Veterans and families on community resources, advance directives and VA benefits. Act as a consultant to other team/staff members. Provide consultation/education to medical residents on social work practice and VA policy and procedure. Act as liaison between VA and community resources, to include marketing and public relation duties as needed. Provide case management to Veterans and families, as needed, throughout the continuum of care. Provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. Address Advance Directives in accordance with the standard of their work areas. Provide wellness/prevention education and facilitate Patient/family support groups as appropriate. As appropriate, supervise the practice of social work students, or a less experienced social worker. Coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans. Participate in committee assignments, as deemed necessary by the service manager. Meet Joint Commission regulations and VA and medical center policy and procedures. Document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. Participate in rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs. Work Schedule: Mon-Fri, 8:00AM-4:30PM Telework: Not Available Virtual.
